Entourage effect

Updated:

Definition

The entourage effect is the hypothesis that cannabinoids and terpenes act together, so a whole-plant extract behaves differently from its isolated parts. Russo's 2011 review made the case from preclinical data. It is the main argument for full-spectrum products, and controlled human evidence for it remains limited.

The phrase entourage effect was coined by researchers studying endogenous cannabinoids, who noticed that inactive related molecules enhanced the activity of 2-AG. Ethan Russo extended the idea to plant compounds in a 2011 review in the British Journal of Pharmacology, arguing that terpenes and minor cannabinoids shape how THC and CBD act (Russo 2011).

The review's examples are mostly mechanistic. Beta-caryophyllene binds CB2 receptors. Pinene inhibits an enzyme that breaks down acetylcholine. Limonene and linalool show activity in rodent behavioral assays. CBD itself changes how THC behaves at CB1, which is the best documented interaction (Russo 2011).

What the hypothesis lacks is controlled human trials that compare a full extract against matched isolated compounds. Reviews of the endocannabinoid system describe a network of receptors, ligands and enzymes where such interactions are plausible (Lu and Mackie 2016), but plausibility is not proof.

In the hemp market, entourage effect is the standard argument for full-spectrum and broad-spectrum products over isolate. It is a reasonable hypothesis to describe, and an overreach to state as settled fact on a label.
